Comprehensive Policy

The disclosure form is available in the Wake Information Network (WIN). The following instructions will step you through the process.

• Log into WIN (for questions about logging into WIN call 758-help)
• Click on "Info Central"
• Click on "Forms and Documents "
• Click on "University Compliance Office"
• Click on "Conflicts of Interest Disclosure Form"
• After completing the form, click submit at the end of the form.

Your disclosure information will feed securely into the University Compliance Office and then be routed back to your Reviewer. If you have any questions or comments, feel free to contact Mark Good.
